What is ImagineCare?

ImagineCare offers a solution for remote patient monitoring, also called distance- or home monitoring, a digital healthcare service proactively measuring your values at home. You will get access through your caregiver, e.g., a specialist or primary care center, to measure your values daily through a Bluetooth-connected medical device, e.g., a blood pressure monitor and a smartphone app. Results are visible immediately for both you and your caregiver. By regularly following up on your results, healthcare staff can offer immediate support in the event of a deviation.

Our patient app, ImagineCare Health Monitoring, is available on AppStore and Google Play.

How do I know if my caregiver offers home monitoring?

As of today, caregivers within the regions of Östergötland and Jämtland Härjedalen offer the service to their patients. If you live in any of those regions or are a patient at Hedda Care or Hedda Hela Livet, contact your caregiver to create your health plan and start the remote monitoring. If not, you can fill in a form at, and we will let you know once ImagineCare is available where you live.

Does the service work on all phones?

ImagineCare’s app, ImagineCare Health Monitoring, is available for iOS and Android, and it works on both smartphones and tablets. Always update the latest app and operating system versions for the best user experience.

What can I be monitored on?

Today, patients can be monitored on several so-called health flows; see examples below. What flows are available differs between different regions and caregivers, so we advise you to talk to your caregiver to discuss an available plan suited to your needs.

What devices and equipment do I need?

Except for a Bluetooth-connected smartphone or tablet, you may need medical devices. Which one depends on what health flow(s) you are connected to. Your caregiver will ensure you get the suitable devices, e.g., a Bluetooth-connected blood sugar or blood pressure monitor, a pulse oximeter, a spirometer, or a scale. You can connect your app to Apple Health or Google Fit to automatically register your steps and sleep.

How do I connect my device?

In our Get started guide (only available in Swedish for now), you can read more about how to connect your device. You can also find step-by-step instructions in the ImagineCare app.

I have issues with my device

There are a few easy steps to follow if you have issues with your device. First, restart the app; if that doesn’t work, try restarting your smartphone/tablet. If you still have issues, follow the below steps:

  1. Disconnect the device from your smartphone/tablet in phone/tablet “Settings,” then “Bluetooth.”
  2. Go back to the app, click on Profile – My devices, and choose the device you want to disconnect. Choose Remove device to clean the device information from the app
  3. Connect your device again by choosing “Connect devices” in the app

If you’re unsure how to connect the device, there are instructions in our Get started guide (only available in Swedish for now) in the app. Go to Profile – Settings – Support – Manuals.

If the device doesn’t start, control that the batteries are put in correctly and change them to new ones if needed.

How do I register my values?

Values such as blood pressure, weight, or blood sugar can be registered manually or automatically through Bluetooth-connected medical devices. In our Get started guide (only available in Swedish at the moment), you can find a step-by-step guide to registering values. You can also follow the instructions in the app.

My values don’t get registered in the app, why?

For the units to communicate, the device must first connect to your smartphone or tablet. Make sure you have Bluetooth activated on your smartphone or tablet, and be aware that Bluetooth has a limited reach, which is why the units shouldn’t be more than a few meters away from each other while connected or in use.

What questionnaires do I need to respond to?

Which questionnaires you need to respond to depends on what health flow(s) you are connected to, but you will always see in the app if you have a new one to handle.

Who do I contact to get help connecting my device or logging into the app?

Who do I contact for questions concerning my health?

Questions concerning your health, e.g., a deviated value, needs to be handled by your caregiver. The easiest way is to write in the app’s chat function.

How do I know my personal information is safe with you?

All information you give in the app, including personal information and measured values, will be part of your caregiver’s medical record, which is confidential according to the law (Patient Data Act). Personal information will also be saved at ImagineCare since the Patient Data Act requires us to. We always value your integrity and don’t collect more info than needed. How do I cancel my monitoring?

Please get in touch with your caregiver to cancel the service.
